Do Foreigners Need Health Insurance in Spain?
Yes. All foreigners applying for a Spanish visa or residency permit must have private health insurance from a carrier authorized to operate in Spain. This applies to every visa category:

What Type of Health Insurance Do Foreigners Need?
Spanish consulates have strict requirements for health insurance. For visa applications, your policy must meet these criteria:
Your policy must provide full comprehensive coverage with no copayments (sin copagos), no waiting periods (sin carencias), and no coverage caps (sin limites). Health Insurance Options by Nationality
US Citizens
Medicare does not cover healthcare in Spain. US citizens must purchase Spanish private health insurance for any visa or residency application. The good news: Spanish insurance is significantly cheaper than comparable US coverage, typically 50-80 EUR per month for those under 50.
UK Citizens (Post-Brexit)
Since Brexit, UK citizens are no longer automatically entitled to Spanish public healthcare. If you have an S1 form (exported UK healthcare rights), you may access the public system. Otherwise, private insurance is required for residency. The GHIC (Global Health Insurance Card) only covers temporary visits, not residency.
EU/EEA Citizens
EU citizens can access Spain’s public healthcare system by registering with their local health centre and presenting their European Health Insurance Card (EHIC) or country-specific documentation. However, many EU expats still choose private insurance for faster access to specialists and English-speaking doctors.
Canadian, Australian, and Other Nationalities
Citizens from countries without reciprocal healthcare agreements with Spain must purchase private health insurance. The process is the same regardless of nationality — choose a Spanish-authorized carrier, get a visa-compliant policy, and obtain an insurance certificate for your consulate appointment.
